All referrals must use the current referral form as of April 1, 2026.
All referrals now require that patients have their Beighton score assessed using the HAT app. HAT app results MUST be attached to the referral package to be triaged

Wait times:
The GoodHope EDS clinic is the only clinic of its kind in Canada. The clinic is only able to accept referrals for residents of Ontario with a valid OHIP card. Due to exceptionally high volume of referrals, current wait times for initial assessment are up 18-24 months.
Referral Checklist
1 / Referral package completed in full
-Reason for referral noted, checklist completed, referral signed and dated by referring provider, additional documentation attached as needed, PCP name and contact information documented
2 / Primary Care Provider information included
- Patients must have a primary care provider in order to be seen in the GoodHope EDS program.
3 / HAT app results attached
- Patient must download the HAT app using the QR code found on our website or in referral package
-Patient must complete the HAT app to assess their Beighton score
- Patient's Beighton score as assessed on the HAT app will be emailed to the patient by the app
- Patient must send the results from the HAT app to their PCP
- PCP must attach the HAT app results to the referral package
4 / 2D echocardiogram results attached
- The results of a 2D echocardiogram must be attached to the referral.
-Referrals without 2D echocardiogram will not be triaged.
5 / Fax complete package to 416-340-3792
- Patient will be contacted directly for an appointment.
- Will take up to 12-18 months to contact patient for initial assessment
